New Patient Registration
To register you will need to contact or call into the surgery and inform us that you wish to join our patient list. You will be eligible to register if you live within our Practice area (click here to find out)
You will be required to complete the necessary application forms. You may also complete and sign a registration application on behalf of family members, or for anyone who has authorised you to do so. You will be required to provide photo identification and proof of your current address.
We will request you to complete a New Patient Questionnaire to provide us with a brief health overview prior to receiving your medical records from your previous practice. Patients are registered with a named doctor and you will be advised of who your doctor is at the time of registration.
Change of Personal Details
If you change your name, address or telephone number please give details to reception. If you move outside the practice area, you will be asked to find a doctor in your area of residence.
