New Patient Registration
It is essential that you live within our practice catchment area to register with the surgery. Please check your address on our map before completing forms. Click here to find out if you are within the practice boundary for Old School Surgery and Alfriston.
Online Registration
We are using a new online service called Register with a GP Surgery that makes it quicker and easier to register with the Surgery.
Just fill in the quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and make it easier for you to register.
Register in person
Paper forms are still available if you need them however please note we will still require proof of address and ID if these are used rather than the online Register with a GP Surgery service. The registration form is available to print at home from the link below or can be collected from reception.

Please do not leave any questions unanswered on the form but enter N/A if the question is not applicable to you, therefore ensuring we can put your correct medical history on our computer.
Paper forms can be returned to the surgery in person, please bring with you proof of address and ID where possible (not required for patients with special circumstances).
When will registration be completed?
Once all the form has been received correctly filled in, registration will take 10-14 working days. We will be unable to supply medication until registration has been completed.
We would like to inform our patients that we record, store, and may monitor or use any incoming and outgoing calls, email or any other communication with you for training purposes and to improve the quality of our services. Calls are stored on a standalone recording system and are not accessed unless there is a training or monitoring need. Change of Personal Details
If you change your name, address or telephone number please email to the surgery, or update via your NHS app. If you move outside the practice area, you will be asked to find a doctor in your area of residence.
